Dear Patient:
We are delighted that you have chosen El Paso Rheumatology for your care and look forward to your visit.
In order to see Dr. Gutarra, all new patients must be referred by their PCP or Specialist. Please ask your doctor to fax your referral to our fax number (866) 611-9943.
Please arrive at least 15 minutes before your appointment time so that we have enough time to process your paperwork.
For future follow-up appointments, please arrive 10 minutes before your appointment time. To expedite our registration process, please complete the attached documentation prior to your appointment.
You must have had your labs and/or X-ray already done at least one week before your follow-up appointment.
When you arrive at our office, please present your completed documentation, photo identification, and your insurance cards.
If your insurance plan requires a referral, it is your responsibility to contact your primary care provider and ensure that they have sent our office a valid referral. We may not be able to see you if there is not a referral on file in our office prior to your scheduled appointment date. Ask your PCP to fax your referral.
After 15 minutes late we will have to reschedule your appointment.
For your convenience, we accept cash, personal checks, Master Card, Visa, American Express and Discover Card.

Failure to appear at the time of a scheduled appointment will be recorded in your medical record as a “no-show” and you will be charged $35.00 on your next visit.
A “no-show” is someone who misses an appointment without notifying the office at least one business day in advance.
Patients are expected to keep their scheduled appointments.
To respect the medical needs of other patients, please be courteous and call the office immediately if you cannot keep a scheduled appointment.
If you need to cancel or reschedule your appointment, please contact our office at (915) 642-9595 no later than 24 hours before your appointment time. Same day Cancelations will be charged $35.00 on your next visit. (Exception: Notification for a Monday appointment must be given no later than 12:00 p.m. on the Friday before your appointment)
IMPORTANT NOTE
If you are a current patient and you want to stay with us but need to change your health insurance coverage – please let us know IN ADVANCE – so that we could help you to make an informed choice. In any case you have to notify us about ANY changes in your Health Insurance coverage BEFORE the care is provided. Otherwise you may be personally liable for the fees incurred
